Greater Holy Temple Christian Academy is a private school that participates in the Milwaukee Parental Choice Program. We are only able to accept applications during our open enrollment periods. Our open enrollment periods are as follow:
February 1-20, March 1-20, April 1-20, May 1-20, June 1-20, July 1-20, August 1-20, September 1-14, October 1-20, November 1-20, December 1 – January 9, 2015
To complete Greater Holy Temple Christian Academy’s Admission’s process, you can come in during office hours and pick up the application or it can be downloaded from the Department of Public Instruction website http://sms.dpi.wi.gov/mpcp_apps.
You must also provide the following items with your completed application in order to have your application request processed:
- Income verification (from Previous Year): Federal Income Tax Return, W-2 Statement, or State Assistance Statement
- Current Address Verification: Cable bill, Gas bill, Electric bill, Home phone bill, Lease Agreement, State Assistance Statement, Pay Stub
- Most recent Report Card
- Copy of student's immunization records
- For K4-1st Grade: Birth Certificate (this must be the original, not a copy)
